The Health Coach Assistant provides clinical and patient engagement
support services within an integrated care model serving HIV-positive and HIV-negative patients. The role is responsible for performing
delegated clinical duties within scope of certification and supporting care coordination activities to promote adherence, prevention, and
continuity of care. All duties are performed in accordance with applicable federal, state, and organizational regulations, policies, and
procedures.

SUPERVISORY AUTHORITY:

Involves no responsibility or authority for the direction of others.

NATURE AND
SCOPE:

External contacts include a variety of third-party payers, medical organizations, patients, and other healthcare
personnel.

PRINCIPAL ACCOUNTABILITIES:

  • Prepare patients for examination by rooming patients and obtaining and documenting
    vital signs, medical histories, and relevant health information.
  • Perform clinical duties within scope of certification, including
    administering injections and immunizations as ordered by a licensed provider (including HIV and PrEP injectables).
  • Conduct
    point-of-care testing in accordance with established protocols.
  • Process, label, and prepare laboratory specimens following infection
    control and safety standards.
  • Perform medication reconciliation and report discrepancies to the provider.
  • Document all
    patient interactions, services, and procedures accurately and timely in the electronic health record (EHR).
  • Adhere to infection
    control standards, OSHA requirements, and universal

  • Reinforce provider-directed care plans and patient education.
  • Assist with appointment scheduling, follow-up
    reminders, and retention initiatives.
  • Support referral coordination and facilitate patient access to community-based
    services.
  • Collaborate with Health Coach(es), medical providers, and interdisciplinary team members to monitor adherence and support
    prevention strategies.
  • Promote continuity of care across services within the integrated care program.

    standards.

MINIMUM REQUIREMENTS:

Current
Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification required. Certification must be maintained in good
standing throughout employment. Current Basic Life Support (BLS) certification required or obtained within organizational timeframe.
Knowledge of medical terminology and clinical procedures. Experience with electronic health records (EHR)
systems.
