Job Summary
The Clinical Pharmacist I optimizes patient care by providing appropriate, safe and cost effective pharmaceutical care including drug dispensing, IV admixture program, and providing drug information. The Clinical Pharmacist I works under the direction of the Director of Pharmacy and/or the Pharmacy Operations Manager and will oversee the work of pharmacy technicians, students, and support staff

Major Responsibilities/Duties/Critical Tasks

  • Functions in accordance with standard written policy & procedures, guidelines, and state and federal regulations.
  • Communicates with other pharmacy team members and caregivers in an effective and positive manner. Demonstrates emotional control and intelligence in managing work relationships.
  • Attends and actively participates in team huddles and meetings.
  • Answers phones in a professional and pleasant manner.
  • Interprets orders and transcribes or verifies them accurately and efficiently in the pharmacy computer system.
  • Accurately compounds and dispenses pharmaceuticals.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
  • Provides accurate and timely medication information to healthcare staff and patients.
  • Effectively and proactively interacts with medical staff regarding drug therapy issue resolutions.
  • Documents all clinical activities and interventions accurately and completely.
  • Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
  • Supervises and assists pharmacy technicians and other support staff effectively.
  • Trains and participates with Disaster Team drills and real-event response as assigned.
  • Prioritizes work and leads others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency.
  • Actively participates in staff meetings and attends staff development in pharmaceutical care to identify learning and performance needs.
  • Supports in the training and mentorship of new staff members as assigned.
  • May serve as preceptor for pharmacy students and/or residents.
  • Supports and participates in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
  • Maintains professional competency sufficient to meet current and future needs of the facility.
  • Maintains appropriate controlled substance records.
  • Complies with all regulatory standards.
  • Performs other duties as assigned.

Required Education/Experience

  • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
  • Doctor of Pharmacy from an ACPE-accredited college of pharmacy preferred.
  • 1 year inpatient hospital pharmacy experience preferred.

Required Certifications/Licenses

  • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
  • Other applicable state requirements i.e. sterile compounding, etc
  • Must be able to obtain and maintain current BLS certification.
  • Must be able to obtain and maintain current ACLS certification.

The retirement plan for this position is Teacher Retirement System of Texas (TRS), subject to the position being at least 20 hours per week and at least 135 days in length.
