THIS IS A GRANT-FUNDED POSITION FUNDED UNTIL DECEMBER 31, 2026.
JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: The Health Coach assists with health screening and health coaching using motivational interviewing techniques to support participants at the Health Hubs in the neighborhood Health Hub program. This position collaborates with clinicians and participants to design individual programs as well as make referrals to necessary primary and behavioral health care and other community supports.
Responsibilities
- Establishes trusting relationships with participants to encourage engagement with individualized programs and healthcare providers for improved health outcomes.
- Assesses and addresses barriers to access preventive care.
- Collaborates with medical professionals and program leads to create educational materials for participants.
- Educates/coaches program participants on disease prevention and advocacy for improved health care.
- Coordinates services with internal and external behavioral and mental healthcare providers.
- Conducts virtual and/or in -person follow-ups to promote continued engagement and retention.
- Generates referrals to community supports and works with community organizations and rental offices to recruit program participants.
- Collects and enters data into the database.
- Assists with the front desk as needed.
- Performs other duties as assigned.
Qualifications
EDUCATION: High School Diploma or GED. (Associate’s or Bachelor’s Degree in Health, Social Services, Human Services, or a related field is preferred.) (TRANSCRIPT REQUIRED)
EXPERIENCE: Two (2) years of related work experience. Must have a valid Driver’s License. (Experience as a Community Health Worker or related job specializing in chronic disease management or prevention related to obesity is preferred.)
KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to communicate effectively with program participants, healthcare providers, and community partners.
- Ability to provide effective education and instruction to program participants.
- Ability to assess the needs of participants and respond appropriately.
- Ability to influence and coach individuals.
- Ability to work in Microsoft Office Suite and virtual platforms.
- Ability to be self-motivated, tactful, and professional both in-person and virtually.
- Ability to relate to individuals working to manage or prevent chronic illnesses related to obesity.
- Excellent interpersonal, verbal, and written communication skills.
- Excellent organizational and time management skills.
WORK SCHEDULE: This position may frequently be required to work weekends and evenings. May require occasional overnight travel.
